Fill in a form

Written for: Pupils.

Sometimes your school asks you to fill in a form, like a survey or a question about a school trip. Your forms are on the Forms page in your Student Radar space.

Find your forms

Section titled “Find your forms”

In the menu, click Forms. Under Forms to complete, each form has its own card. A card shows:

  • the name of the form
  • what it’s for
  • whether you’ve started it or finished it
  • when it Closes, or when it Opens if it isn’t open yet

If you see No forms waiting, you don’t have any forms to fill in right now.

Fill in a form

Section titled “Fill in a form”
  1. Click Open form on the form’s card.
  2. Read the questions and answer them.
  3. When you’ve answered everything, click Submit form.

You’ll see Form submitted. That means your school has your answers.

While you’re filling in a form, your answers are saved as you go. You’ll see Saving… and then Saved at the bottom of the form. So if you need to stop, you can come back and carry on later.

To go back to your list of forms, click Back to forms.

Change your answers

If your school lets you change your answers, you can open the form again after you’ve submitted it. Change your answers, then click Submit amendment.

To look at a form you’ve already finished, click View response on its card.

Anonymous forms

Section titled “Anonymous forms”

Some forms are anonymous. Your answers are saved without a record of who filled them in. These have an Open anonymous form button. When you’ve finished, click Submit anonymous response. You’ll see Anonymous response submitted.

If something doesn’t work

What you see What to do
“This question is required” You need to answer this question before you can submit the form.
Not open yet on the card The form isn’t ready yet. Look at when it Opens and come back then.
Closed on the card The form has closed. If you still need to fill it in, ask your teacher.
“This form is closed” The form has closed, so you can only read your answers.
“Changes could not be saved. Keep this page open.” Don’t close the page. Wait a moment, then carry on. If it keeps happening, tell your teacher.
“Forms could not be loaded” Click Try again.
