Connect / Forms and Consent
Forms covers consent requests, applications, surveys and data collection. A form is built as a draft from a template, published as a fixed version, and sent out in a campaign with an opening and a closing time.
The school decides what a form asks, who approves it and who reads the answers. Forms records what was published, sent, submitted and reviewed.
From draft to answered
New consent requests, including trip consent, arrive in Forms alongside surveys, applications and data collection sheets.

Start from a shared template or a ready-made starter, set the purpose and data classification, and add questions with types, required answers, help text and conditions. Preview it as people will see it.
02
Publishing fixes the version people receive. Forms that collect consent, a signature, or medical or financial details need a colleague with permission to publish sensitive forms, and a form can require approval of its exact revision first.
03
A campaign sends one published version to chosen families, pupils or staff, with a completion policy, a rule on amendments and a closing time. The response view shows who has opened, submitted or not yet answered.
Families and pupils
Parents open a form in the parent app or from the school’s email, save as they go and sign where the form asks. For consent, declarations and sensitive information, a six-digit code sent to the address the school used checks that it is really them. Pupils have their own Forms page in their Student Radar space.
Families need a parent app account first.
Campaign recipients are chosen by pupil, up to 200 at a time, and pupils whose parents have no parent portal account cannot be chosen until they do.
Approval and review
Where the form owner asks for it, each response waits for a colleague to approve it, request changes or reject it, with a rationale. A review never changes what was submitted, and nobody approves their own form or their own response.
The published version, who it went to, who answered and what was reviewed stay together, so the office is not reconciling replies by hand.

Approval covers the exact revision that was read, and a later edit needs approving again.
The school can see the original answer and the withdrawal or amendment that followed.

The value of a connected school
Family communication becomes more useful when the information, response and next school action stay connected.

First aid and medication records keep parent notification status with the event. Staff can see where follow-up is needed without reconstructing a separate message trail.
