Complete a form or consent request

Written for: Parents and carers.

Schools use forms to ask parents and carers for information or permission, for example consent for a school trip, a photo consent form or a data collection sheet. You can fill them in from the Forms page in the parent portal, or from the link in the email the school sends you.

  1. In the menu, click Forms.
  2. Under Forms to complete, look at Assigned forms. For each form you’ll see:
    • its name
    • For: which child it’s about, or Household if it’s for your family
    • Purpose: what kind of form it is, such as consent
    • Status: whether you’ve started or finished it
    • Closes: the deadline. If the form isn’t open yet, you’ll see when it Opens instead.
  3. Click Open form next to the form you want to fill in.

If you see No forms waiting, there’s nothing for you to fill in at the moment.

The school’s email has a button that opens the form. You don’t always need to sign in.

For a form that records consent, a declaration or sensitive information, Student Radar first checks it’s really you:

  1. On Verify before continuing, click Email me a code. The code is sent only to the email address the school sent the form to.
  2. Find the email with your code. The code expires after 10 minutes.
  3. Type the code into Six-digit access code.
  4. Click Continue securely.

If the code doesn’t arrive or has run out, click Send a new code. School staff will never ask you for this code.

Some schools ask you to sign in to the parent portal before you fill in a form. If you see Portal sign-in required, click Sign in to continue. See Sign in to the parent portal.

  1. Read the questions and answer them.
  2. Your answers save as you go. You’ll see Saving… and then Saved at the bottom of the page, so you can stop and come back later.
  3. When you’ve answered everything, click Submit form.

You’ll see “Form submitted”. The school now has your answers.

Some forms ask you to sign:

  • Type your full name in the signature box.
  • Tick I intend to sign and accept this declaration.
  • If the form asks for a drawn signature, draw it in the box, then click Save drawing. Click Clear drawing to start again.

If the form can be read in other languages, choose one under Form language at the top. A translated form is a translation of the school’s English form, which is the official version.

If the school allows changes, you can open the form again until it closes. Click View response in your list of forms, change your answers, then click Submit amendment.

For a consent form, you may be able to withdraw your consent:

  1. Open the form and scroll down to Consent controls.
  2. Click Withdraw consent.
  3. Type a short reason in Reason for withdrawing consent.
  4. Click Confirm withdrawal.

You’ll see “Consent withdrawn”. Your original answer is kept, and the withdrawal is added after it, so the school can see both.

Some surveys are anonymous. They’re listed under Anonymous forms, with Open anonymous form. You may be asked to sign in so Student Radar can check you’re one of the people the school asked, but your answers are saved without your name. Avoid writing names or other personal details in your answers unless you want to be identified. When you’ve finished, click Submit anonymous response.

New consent requests from school, such as trip consent, arrive in Forms. The Consents page keeps the consent decisions you made before your school moved consent requests to Forms. If you see “New consent requests are now in Forms.”, click Open Forms.

For medication, healthcare plans and allergy consent, the school may also send you a separate link. See Authorise medication or acknowledge a health plan.
