What it does
Concern capture takes what a member of staff noticed and records it, with the detail structured so the DSL can act on it.

Staff do not need proof to record a concern. Noticing is enough; deciding is the DSL's job.

An urgent concern is still reported verbally to the DSL. The record follows the words; it does not replace them.

Recording a concern starts a record, not a case. What happens next is a decision, made by a person.
