EHCPs: connect the source plan with outcomes and provision

Student Radar helps SEND teams read uploaded EHCPs, review versions and work on Section E outcomes and Section F provision, checking the organised content against its original source.

A prepared working copy is not the issued original. Extracted steps, confirmed provision, map links and delivered support are different evidence and require human review.

Check the intended pupil, document state and review/source information. Fictional school example.

EHCPs

From the recorded source to useful support

Connect the uploaded EHCP, prepared sections, review context, outcome evidence and confirmed provision without losing the source.

  1. 01

Open the right source and version

Find the pupil, check need/review provenance and open the organised sections. Compare the working wording with the uploaded original, especially when a newer upload is still being prepared.

  2. 02

Turn outcomes and provision into reviewable work

Review Section E small steps and Section F items against the source. Check proposed wording, who/how much and the link to outcomes before confirmation; inferred quantities or costs remain subject to review.

  3. 03

    Follow the permitted next action

Open the current Annual Review, passport, outcome work or provision map. Check confirmation counts and subsequent delivery evidence separately. A letter draft or downloaded PDF is not external delivery.

Keep the original within reach

The plan overview names the uploaded source and version beside review and coverage context. Staff can move through sections and open the original page when a passage needs checking.

Check the wording

Read the wording before interpreting it

Section navigation and document search help staff locate the relevant needs and provision. An unavailable extraction is a limitation, not proof that the original contained no such section.

Review deliberately

Check a specific provision item

The Section F workspace keeps the source quote beside who, group, amount and outcome links. Checks flag missing specificity or quantities; the professional decides what to confirm or raise.

Keep the states distinct

Keep confirmation and mapping distinct

The fictional provision reopens as confirmed with a separate Add to map control. A confirmed item or provision-map entry does not establish that the child received the support.

Questions about EHCPs

Can staff work on Section E and Section F separately?

EHCPs includes Section E outcome work and Section F provision review, with deliberate confirmation, mapping, rejection or issue-raising steps.

Can staff inspect EHCP versions and the uploaded original?

The EHCP workflow includes plan reading, versions and downloads, with upload or replacement. Organised or extracted content still needs checking against the original source.

The value of a connected school

A plan matters when support reaches the child.

Bring the evidence around provision into the review, so the SENCO and teaching team can decide the next step together.

Follow a connection to see what it makes possible

Relevant context, with access set by each staff member’s role and responsibilities. Staff decide the response.

  1. Attendance + SEND

    Check attendance alongside planned support

Look at absence and late arrival around provision. A pupil may have a plan in place and still miss opportunities to receive that support.

  2. Assessment + SEND

Bring progress evidence to the review

Consider recorded learning and small steps alongside targets and provision. Keep the evidence visible while staff judge what is helping and what needs another look.

  3. Family communication + SEND

Include the people who know the pupil

Gather family contributions and coordinate review conversations around the same pupil, with named actions and dates to keep the agreed work moving.
