Alternative Provision / AP Risk Assessments
AP risk assessment drafts, approval and review history
Student Radar helps alternative provision staff record hazards, controls and professional judgements, then obtain the appropriate review and approval. The current signed assessment stays available while an update awaits review; a saved assessment does not establish that an arrangement is safe.
Staff decide whether the assessment and controls are suitable. A residual score, saved draft or approval records a judgement; none is a guarantee that an activity or arrangement is safe.
From drafted hazards to a reviewed current version
Review the risk record without erasing the signed version.
Draft pupil assessments, record hazards and controls, and keep the current signed assessment available while an update awaits its authorised review.
01
Identify the actual subject
Choose the pupil or reusable site/activity workflow and check current access.
02
Record and review the controls
Describe hazards, people affected, dates and professional judgement.
03
Maintain the right live version
Approve through the correct authority and review updates without overwriting signed history.
Two assessment subjects with distinct authority
Two assessment subjects with distinct authority
Individual pupil assessments and reusable site/activity assessments have different lifecycle and permission checks. A site/activity approver must be neither the owner nor anyone who saved or submitted its content since the last approval.
Keep coverage gaps visible
Keep coverage gaps visible
Current roll coverage distinguishes live, due, overdue, draft-only and no-assessment states. An unavailable source is not a reassuring zero.
Separate scores from the overall judgement
Separate scores from the overall judgement
Record named hazards, who is affected and controls with the actual dates. Likelihood × severity supports discussion; staff choose the overall rating.
Keep drafting distinct from approval
Keep drafting distinct from approval
Reopening a saved draft shows who prepared it and what still needs an authorised sign-off. Drafts do not count as live cover.
Draft an update while preserving the signed record
Draft an update while preserving the signed record
Editing a live assessment creates a separate review draft. A waiting draft is reused rather than silently creating parallel updates.
Read both versions in history
Read both versions in history
The original remains Live while the update awaits sign-off. Approving the replacement supersedes the earlier version with history retained.
Use the reusable workflow deliberately
Use the reusable workflow deliberately
Sites and activities use Trips-backed drafting, submission and an independent approval from someone who did not own, save or submit the version since the last approval. Linking an approved assessment does not approve an entire trip or obtain consent.
Read the same state on a phone
Read the same state on a phone
The narrow screen keeps live coverage, waiting drafts and pupils without a live assessment as separate counts.
Make the next decision from the evidence
Staff decide whether the assessment and controls are suitable. A residual score, saved draft or approval records a judgement; none is a guarantee that an activity or arrangement is safe.
A current signed version retained while an update is reviewed
A clear distinction between a pupil record, reusable assessment and trip approval
Questions about AP Risk Assessments
Can we retain hazards and controls in a draft assessment?
Yes. Draft the pupil assessment with its hazards, controls and professional judgements before the authorised approval decision.
Can staff see the signed assessment while an update is reviewed?
Yes. The current signed assessment remains available while its update awaits review, preserving the earlier history.

Raise a concern from the pupil you are working with
Provider staff can raise a concern, and authorised DSLs work it on the DSL Action Board, with the chronology and record transfer in the same place. Attendance, review and timetable views shared about a named learner leave out protected record types. Staff see summaries and only the records they are separately authorised to open, and a link does not give wider access.

See the placement from the commissioning school’s side
Schools that commission placements keep a provider register with due-diligence checks, placement checks before, during and after the placement, and an evidence pack they can export.
