Alternative Provision / AP Placement Reports

AP placement report drafting and leader sign-off

Student Radar helps alternative provision staff prepare a snapshot of selected placement evidence, write a professional summary and explain next steps and evidence gaps. An authorised leader signs off the report before any separate publication to a school.

Staff write and approve the review. A high percentage does not remove missing marks; signing or downloading a report does not publish it or prove a placement caused improvement.

Seven of eight counted sessions are present; two of ten planned occurrences still lack a mark.

From bounded evidence to an approved review

Report the evidence. Keep the judgement human.

Prepare a placement snapshot, explain the observed response and evidence gaps, and obtain a leader’s sign-off before the separate school-publication step.

  1. 01

    Choose the real reporting scope

Select pupil, one placement where sharing is intended, kind and dates.

  2. 02

    Write the professional interpretation

Read the source window and gaps, then record the supported summary and next checks.

  3. 03

    Approve and share deliberately

Confirm the saved revision, sign off with authority and publish through its placement separately.

A snapshot with its limits intact

The pack preserves source versions and coverage. Current accreditation detail stays explicitly separate from the signed snapshot, while legacy missing coverage remains unknown.

Prepare for one pupil and one agreement

Prepare for one pupil and one agreement

The reporting picker uses permitted current or historical learners. Choose one overlapping placement when the result needs to be published to a school.

All placements in period is a useful overview but cannot be published as one placement’s report.

Say what the observation supports

Say what the observation supports

Write the human summary and practical next steps. Recording a response with a prompt is different from claiming sustained change or a completed follow-up task.

The example retains two missing marks and a specific check for the next review.

Make the decision attributable

Make the decision attributable

An authorised leader reviews the statement, source snapshot and current report revision. A text save can succeed even if approval is then refused.

The statement records the bounded review; it is not a guarantee of complete evidence.

Keep approval and publication separate

Keep approval and publication separate

Reopening the signed pack shows locked text and the separate placement sharing route. A download is a file for authorised use, not delivery to a school.

School publication still needs Communication and access on the named placement.

Review coverage on a phone

Review coverage on a phone

Read the same evidence window, source counts and missing marks before interpreting the summary or approving the report.

The narrow screen preserves the gap rather than hiding it behind a headline percentage.

Make the next decision from the evidence

Staff write and approve the review. A high percentage does not remove missing marks; signing or downloading a report does not publish it or prove a placement caused improvement.

A report tied to a pupil, period and source evidence

A human review that preserves missingness and next checks

An attributable approval with publication and download boundaries clear

Questions about AP Placement Reports

Can a placement report combine a snapshot with professional commentary?

Yes. Prepare the bounded record snapshot, then write the observed response, evidence gaps and proposed next steps for review.

Can a leader review and sign off before the school sees a report?

Yes. Authorised leader sign-off precedes the separate school-publication step. Saving, signing, downloading and publishing are distinct actions.

The value of a connected school

A placement touches more than the timetable.

Provider staff use the same safeguarding and health tools as mainstream schools, and commissioning schools on Student Radar have their own side of the placement.

Follow a connection to see what it makes possible

Relevant context, with access set by each staff member’s role and responsibilities. Staff decide the response.

  1. Safeguarding + Alternative Provision

Raise a concern from the pupil you are working with

Provider staff can raise a concern, and authorised DSLs work it on the DSL Action Board, with the chronology and record transfer in the same place. Attendance, review and timetable views shared about a named learner leave out protected record types. Staff see summaries and only the records they are separately authorised to open, and a link does not give wider access.

  2. Health + Alternative Provision

Check medical needs before an off-site session

Authorised staff can check medical needs, medications and healthcare plans for the young people in the provision before a session, and record first aid when something happens.

  3. Commissioning school + Alternative Provision

See the placement from the commissioning school’s side

Schools that commission placements keep a provider register with due-diligence checks, placement checks before, during and after the placement, and an evidence pack they can export.

Ask to see a report from evidence to approved revision

Prepare one placement’s pack, explain its denominator, save the professional review, sign off and show the separate action that would publish it to a school.
