Alternative Provision / AP Physical Intervention Log

AP physical intervention records and senior review

Student Radar helps alternative provision staff retain the factual account of a physical intervention, injuries, debriefs and contact already made. Earlier versions, follow-up and independent senior review stay visible; the workflow does not instruct or authorise a physical technique.

The log records facts and human decisions. It does not authorise a technique, establish a suitable duration, verify training or prove an intervention was appropriate; review must be independent of the recorder and anyone involved across retained revisions.

The entry remains awaiting review; no training clearance, completed contact or approved practice is inferred.

From factual record to an independent review

Keep the facts, follow-ups and independent review connected.

Record an AP physical intervention, retain earlier versions and keep debriefs, notifications and a separate senior review visible in the same restricted source record.

  1. 01

    Record what actually happened

Identify pupil, involved staff, context, reported technique, duration and injuries.

  2. 02

    Complete the source facts

Record actual debrief and contact outcomes, preserving pending and unsuccessful states.

  3. 03

    Obtain an independent decision

Have an authorised uninvolved senior reviewer read the current revision and record the outcome.

A restricted record with distinct authority

A restricted record with distinct authority

The log has its own pupil-scoped read, record and review rights. General Behaviour access or an administrator label does not imply access to these entries.

Name the actual people without assuming clearance

Name the actual people without assuming clearance

Use the SCR identity and distinguish involved adults from witnesses. Planned, missing, expired or later training evidence is not valid training at the intervention date.

The record names an involved adult; no training record is linked in this example.

Record the account without prescribing a response

Record the account without prescribing a response

Keep reasons, antecedents, attempted de-escalation, technique and duration factual. The entered minute count is not an acceptable-use threshold.

The example is a record demonstration, not a technique or risk recommendation.

Leave unknown and unfinished work explicit

Leave unknown and unfinished work explicit

Debrief, parent contact and school contact retain their own states and actual times. Recording a channel does not send a notification.

No one is recorded as told merely to make the senior review available.

Read the denominator before interpreting a count

Trends compare recorded entries with placements and staff on record for the window, keeping outside-scope entries and witnesses distinct.

These recorded counts do not measure need, risk or the quality of practice.

Keep review independent and current

Keep review independent and current

In a separate fictional entry recorded by another colleague, an uninvolved authorised reviewer records the decision after the follow-up facts are present.

An agreed action note is a review record, not every separate support task already created or completed.

Preserve the terminal review and history

Preserve the terminal review and history

Reopening the reviewed entry shows its attributed outcome. Review and Withdraw are no longer offered; a still-recorded error has a separate independent withdrawal path.

Reviewed is a terminal record state, not proof of an educational or safeguarding impact.

Read the same senior decision on a phone

Read the same senior decision on a phone

The phone keeps the entry’s current state, review attribution and retained source work readable before another action is considered.

The same source decision remains readable without expanding access to another pupil’s record.

Make the next decision from the evidence

The log records facts and human decisions. It does not authorise a technique, establish a suitable duration, verify training or prove an intervention was appropriate; review must be independent of the recorder and anyone involved across retained revisions.

A factual intervention account with attribution and retained revisions

Debrief and notification states connected to their source-linked work

An independent terminal review that preserves professional responsibility

Questions about AP Physical Intervention Log

Can we keep debriefs and notifications with the incident account?

Yes. The restricted record connects what happened with debriefs, notification records and follow-up, while preserving earlier versions.

Can a separate senior reviewer review the intervention?

Yes. The workflow includes independent senior review, distinct from factual recording or recording contact already made.

The value of a connected school

A placement touches more than the timetable.

Provider staff use the same safeguarding and health tools as mainstream schools, and commissioning schools on Student Radar have their own side of the placement.

Follow a connection to see what it makes possible

Relevant context, with access set by each staff member’s role and responsibilities. Staff decide the response.

  1. Safeguarding + Alternative Provision

Raise a concern from the pupil you are working with

Provider staff can raise a concern, and authorised DSLs work it on the DSL Action Board, with the chronology and record transfer in the same place. Attendance, review and timetable views shared about a named learner leave out protected record types. Staff see summaries and only the records they are separately authorised to open, and a link does not give wider access.

  2. Health + Alternative Provision

Check medical needs before an off-site session

Authorised staff can check medical needs, medications and healthcare plans for the young people in the provision before a session, and record first aid when something happens.

  3. Commissioning school + Alternative Provision

See the placement from the commissioning school’s side

Schools that commission placements keep a provider register with due-diligence checks, placement checks before, during and after the placement, and an evidence pack they can export.

Ask to see a recorded intervention through independent review

Create a fictional entry with pending follow-ups, inspect why its recorder cannot review it, then review a separate eligible example and reopen the attributed terminal record.
