Alternative Provision / AP Incident Log

AP incident records and commissioning-school contact

Student Radar helps alternative provision staff record observed incidents, make the professional significance decision and retain commissioning-school contact details. The Record report action records contact already made; it does not send a message or complete a safeguarding response.

Staff decide significance and carry out contact through the agreed process. Record report saves a manual account of contact already made; it sends nothing and does not close a safeguarding case or prove an outcome.

The same-day label describes the entered contact date, not an email delivery receipt.

From observed incident to recorded school contact

Record the incident. Keep the reporting fact separate.

Log the observed event, make the professional significance decision and record who was actually told at the commissioning school, with the date and channel retained.

  1. 01

    Record the observed event

Choose the permitted pupil and distinguish observed facts from reported explanations.

  2. 02

    Decide significance deliberately

Record the professional reason, rather than infer severity from a points value.

  3. 03

    Confirm the actual report

Tell the correct school through the agreed channel, then record who was told and when.

One canonical incident, separate reporting evidence

One canonical incident, separate reporting evidence

The AP log consumes the school’s behaviour record. A significance or contact entry does not amend, close or delete that incident, send a family notice or complete separate work.

Keep the judgement attributable

Keep the judgement attributable

Enter the actual category and account, then mark significant only with the reason for that decision.

The app records the decision; it does not automatically classify the event or diagnose motive.

See what still needs recording

See what still needs recording

The saved significant incident names its placement context and outstanding report. Logging can succeed even if the later significance command fails.

Awaiting report means no report recorded here, not proof no contact occurred.

Record the contact that already happened

Record the contact that already happened

Check school, date, time, channel and person. A late report remains late rather than being backdated to clear a prompt.

This form records a call; it does not place the call or send a message.

Read the reporting status with its context

Read the reporting status with its context

The significant view places outstanding reports first while keeping the source record, reason and shared-information note readable.

Same-day reporting is distinct from resolving the incident or completing an agreed next check.

Keep the scope visible on a phone

Keep the scope visible on a phone

The phone retains the chosen section and distinct incident, significance and reporting counts before staff open the named event.

The fictional count is one reported incident; it is not a whole-school safety conclusion.

Make the next decision from the evidence

Staff decide significance and carry out contact through the agreed process. Record report saves a manual account of contact already made; it sends nothing and does not close a safeguarding case or prove an outcome.

An observed event separated from the staff significance decision

A recorded school-contact account with date, channel and person

Clear reporting gaps and source-specific correction boundaries

Questions about AP Incident Log

Can we record who at the commissioning school was told?

Yes. Record the actual contact, with its date and channel, against the incident. The action records reporting already done.

Can staff decide and record whether an incident is significant?

Yes. Staff make that professional decision alongside the observed event. The log does not make an automatic safeguarding decision.

The value of a connected school

A placement touches more than the timetable.

Provider staff use the same safeguarding and health tools as mainstream schools, and commissioning schools on Student Radar have their own side of the placement.

Follow a connection to see what it makes possible

Relevant context, with access set by each staff member’s role and responsibilities. Staff decide the response.

  1. Safeguarding + Alternative Provision

Raise a concern from the pupil you are working with

Provider staff can raise a concern, and authorised DSLs work it on the DSL Action Board, with the chronology and record transfer in the same place. Attendance, review and timetable views shared about a named learner leave out protected record types. Staff see summaries and only the records they are separately authorised to open, and a link does not give wider access.

  2. Health + Alternative Provision

Check medical needs before an off-site session

Authorised staff can check medical needs, medications and healthcare plans for the young people in the provision before a session, and record first aid when something happens.

  3. Commissioning school + Alternative Provision

See the placement from the commissioning school’s side

Schools that commission placements keep a provider register with due-diligence checks, placement checks before, during and after the placement, and an evidence pack they can export.
