Alternative Provision / AP Custom Forms

AP custom forms and published referral-stage versions

Student Radar helps alternative provision teams build and preview custom forms, review staff audience and publication settings, then attach an exact published version to a referral stage. Saving a draft does not publish it, attach it to a workflow or complete it for staff.

Draft saving, publication approval, publication, workflow attachment and response acceptance are separate steps. Staff review the information and retain the specialist decisions in their owning workflow.

The fictional library separates published forms from drafts and shows one form version used by one of two intake stages. Fictional school example.

AP Custom Forms

From a staff question to a versioned stage requirement

Build and preview a provision form, review its staff audience and publication settings, then attach the intended published version to a referral stage.

  1. 01

Build the questions the stage actually needs

Choose a suitable purpose and classification, write the questions and validation, then preview the staff-facing wording and conditional paths.

  2. 02

Save and publish the exact reviewed revision

Keep the draft separate from publication history; review staff audience, identified responses and required approval before publishing another version.

  3. 03

Attach and complete the intended published version

Select the eligible published form version in the workflow stage, publish the workflow deliberately and complete/review the response through the referral’s authorised requirement.

Build the factual record

Write a factual staff record

The fictional general form asks what was discussed. Required answers, field limits and a stable output key define the record; a question is not a substitute for a placement or specialist decision.

Preview the staff experience

Check the wording before people complete it

The fictional preview shows the staff introduction and discussion question. Previewing rehearses the form without submitting a response or publishing new wording.

Review publication arrangements

Keep staff identity and approval explicit

The fictional form includes Staff and identified responses, with publication arrangements and existing version history available separately. Eligibility also excludes payment, specialist links, external webhooks and trusted prefill.

Return to the saved draft

Retain draft changes without replacing the attached version

The reopened fictional introduction retains the source-of-account wording after Save draft. The existing published version1 stays the workflow reference until a separate publication and stage change.

Choose the stage version

Attach a published version through workflow settings

The fictional workflow settings expose stages and form requirements. Choosing Attach forms to referral stages only navigates here; a deliberate version selection and Publish new version are still required.

Questions about AP Custom Forms

Can we preview questions and validation before publishing a form?

Yes. Build sections, fields and validation, then preview the relevant paths and review staff audience and publication settings.

Can a referral stage use a specific published form version?

Yes. Attach the intended published version to its stage. Later draft edits do not automatically replace the version already used by that workflow.

The value of a connected school

A placement touches more than the timetable.

Provider staff use the same safeguarding and health tools as mainstream schools, and commissioning schools on Student Radar have their own side of the placement.

Follow a connection to see what it makes possible

Relevant context, with access set by each staff member’s role and responsibilities. Staff decide the response.

  1. Safeguarding + Alternative Provision

Raise a concern from the pupil you are working with

Provider staff can raise a concern, and authorised DSLs work it on the DSL Action Board, with the chronology and record transfer in the same place. Attendance, review and timetable views shared about a named learner leave out protected record types. Staff see summaries and only the records they are separately authorised to open, and a link does not give wider access.

  2. Health + Alternative Provision

Check medical needs before an off-site session

Authorised staff can check medical needs, medications and healthcare plans for the young people in the provision before a session, and record first aid when something happens.

  3. Commissioning school + Alternative Provision

See the placement from the commissioning school’s side

Schools that commission placements keep a provider register with due-diligence checks, placement checks before, during and after the placement, and an evidence pack they can export.
