Health / Individual Healthcare Plans

Care plans staff can open, with protocols, contacts and review dates inside.

Bring conditions, protocols, contacts, review dates and approved outputs together so care can be delivered consistently.

Health information is available only to authorised staff and supports, rather than replaces, the school's clinical procedures.

The working surface

What it does

An individual healthcare plan here holds the condition, the agreed protocols, emergency contacts and the review date, with an assisted-drafting wizard that produces a draft for staff review.

Limits made explicit

What it deliberately does not do

01

Assisted drafting produces a draft, nothing more. A member of staff reviews it and makes it active.

02

An active plan is the school's working copy of what was agreed with clinicians and families. When the agreement changes, the plan changes when a person updates it.

03

A plan being on file does not make it current. Plans past their review date are labelled overdue, and the list says how many.

Evidence in context

From agreement to the school day

The plan connects what was agreed with clinicians and families to the staff who carry it out, and to the review that keeps it honest.

  • Condition, protocols and contacts in the plan staff open
  • Overdue, due today and upcoming review labels on the list
  • Permission-controlled visibility for sensitive detail
